MPI_Name_get twice specified

I saw that we have used the procedure name "MPI_NAME_GET"
for different purpose in mpi-dynamic (p.58) and mpi-external (p.143).

I propose to use in mpi-external (p.142-144)

MPI_IDENT_PUT instead of MPI_NAME_PUT
MPI_IDENT_GET instead of MPI_NAME_GET
MPI_MAX_IDENT_STRING instead of MPI_MAX_NAME_STRING
identifier instead of name
