New interim collective chapter

The latest collective chapter, which revises the definition of channels,
and eliminates non-blocking collective operations, is available as

Two other action items were defined by straw poll of the subcommittee:
* MPI_COMM_DUP_LOCAL to make come back to help allow users to write
their own non-blocking operations
* A jod chapter, showing how to do non-blocking operations with threasd
is requested.
