As pointed out by Burns, Collective chapter should review all collective
operations in MPI-2, and grade the implied implementation requirements
of these against what is directly posed in this chapter, and considered
for inclusion / omission based on incremental cost to implement.
Dynamic chapter needs to be reviewed, but so do others. One sided is
replicating non-blocking capabilities with assumption that collective
chapter is moribund, for instance...
We need to make a note to restore text on collective cancel, in as much as
the other chapters have these non-blocking collectives.
