New version of P/G/O proposal

I have cleaned up the P/G/O proposal somewhat. Primary differences include:
* An introduction listing some differences from the chapter 4 proposal
* Correction of some very confusing typos
* Addition of some Fortran and C bindings
* Replacement of the improper usage of "request" with "event"

I will include the postscript in the following message. It can also be
obtained at http://www.nas.nasa.gov/NAS/Tools/MPI2/
