MPI_ACCUMULATE

I have only just realized the asymmetry with our accumulate model: we have a
put-and-accumulate, but we do not have a get-and-accumulate. I see no obvious
reason to prefer one over the other, in term of either usefullness or easy of
implementation.

So I would like to propose that as a minimum, we rename MPI_ACCUMULATE as
MPI_PUT_ACCUMULATE, or perhaps MPI_PUT_ACCUM. I would also support adding a
MPI_GET_ACCUM function as well, which we could then do cleanly.
