MPI_RMA_MALLOC now only marks a block of memory as "good", but does not create
a window.
Removed the previous restrictions on target datatypes.
Removed the counters associated with windows to a separate section, to be
discussed as a possible enhnacement.
Definition of Fence changed.
Semantics section changed.
The handler section has changed significantly.
P.S. I am willing to take out the long examples, and save them for the next
edition of my book.
